PI Eligibility Exception Form

An individual must meet certain criteria in order to serve as PI for research with human subjects. Those criteria may be found in the OHR Policy on Principal Investigators (DOC).

To apply for an exception to these PI eligibility requirements, a PI Eligibility Exception Form (PDF) must be completed and submitted to OHR at [email protected], along with the proposed PI's CV, letter of appointment, and any other necessary supporting documents.

OHR will review the request for exception and will email a signed form to the individual when the request is approved, which they should then upload into the Training History under “My Assistant > My Account Information.”

For any study covered by the exception form, the PI must upload the signed and approved PI Eligibility Exception Form as a study document.
